DID YOU RECEIVE A EXEMPTION CERTIFICATE REQUEST?

If you have questions regarding a specific customer account, for example you received an exemption certificate request for XYZ Company, please check your cover letter for the appropriate email address to contact such as XYZ@certcapture.com and the assigned customer service representative will respond to your questions and additional information requests.

ARE YOU LOOKING FOR AN EXEMPTION CERTIFICATE AND IT IS NOT ON OUR BLANK CERTIFICATE PAGE?

We do our best to stay current with new certificates and provide a valuable listing of the most commonly used state exemption certificates. If you have come across a certificate or two that you use, but it is not on our blank certificate table please let us know and we will post it online. Sometimes, we have the certificate, but don't believe people will be looking for it, so we don't post it online. We welcome the discussion with you about what exemption certificates you feel are important.
